BIOIDENTICAL HORMONE REPLACEMENT THERAPY
What bioidentical hormone replacement therapy is, and how we prescribe it
Bioidentical hormone replacement therapy (BHRT) is menopause hormone therapy that uses hormones with the same molecular structure as the ones your ovaries made: estradiol for the symptoms, and micronized progesterone to protect the uterus. Here is what that means in practice, who it helps, and the forms it comes in.
Bioidentical vs. synthetic
“Bioidentical” describes the molecule, not where it comes from. Estradiol and micronized progesterone are bioidentical whether they arrive as an FDA-approved patch, gel or capsule, or in a cream compounded for you by a pharmacy. Synthetic alternatives such as conjugated equine estrogens and medroxyprogesterone acetate act on the same receptors but are not the same molecules; most of the evidence on hormone-therapy risk comes from trials of those older products.
Menopause-society guidance recommends the FDA-approved bioidentical products where they fit, and says that compounded hormone therapy has not been shown to be safer or more effective than approved products. BaselineRx prescribes both kinds, says which is which on every page, and never describes a compounded product as FDA-approved.
Who it helps
Women in perimenopause or menopause with symptoms that get in the way: hot flashes and night sweats, sleep broken by them, mood changes, brain fog, joint aches, vaginal dryness and painful sex. For most healthy women under 60 or within 10 years of the start of menopause, current guidance holds that the benefits of hormone therapy outweigh the risks, and that it remains the most effective treatment for hot flashes and night sweats.
It is not for everyone. A history of breast or uterine cancer, blood clots, stroke or heart attack, unexplained vaginal bleeding, active liver disease, or a possible pregnancy rules it out, and the intake screens for each of these before a clinician ever sees your file.
How BaselineRx prescribes it
A private two-minute intake covers your symptoms, your history and whether you still have a uterus; no labs are needed to start. A US-licensed clinician from Arora Health & Aesthetics, whose clinical team designed the protocol, reviews it and chooses the form, the strength and whether progesterone or vaginal cream is added. Transdermal estrogen is the default, oral tablets are reserved for women who have reacted to topical estrogen, and most women stay on their starting dose, with a review around month three. How online treatment works · Who prescribes
Two hormones are needed to create balance.
Most hormone therapy is a patchwork: an estrogen patch, a progesterone pill, sometimes a separate vaginal cream. Our bundles cover everything you need, as deemed appropriate by your clinician.
01 · ESTRADIOL
The estrogen that drives the symptoms
Bioidentical estradiol addresses the whole cluster: hot flashes, night sweats, sleep, mood, dryness, brain fog.
02 · MICRONIZED PROGESTERONE
The partner that protects the uterus
Anyone with a uterus who takes estrogen needs progesterone. Bundling it removes the second prescription and the second decision.
03 · VAGINAL, TOPICAL OR ORAL DELIVERY
How to decide
Based on your preferences, allergies, or lifestyle one option may be better than another.
These two hormones are available in four formats. During intake you can state your preference or ask the clinician to recommend.
Your clinician reviews your symptoms and history, then builds your kit: the estradiol format, progesterone if you have a uterus, and vaginal cream if you need it.

IMPORTANT SAFETY INFORMATION+
Hormone therapy has risks, including blood clots, stroke, and breast and uterine cancer. These depend on your age, time since menopause and health history, and your clinician will review them with you. It's generally not for women with a history of breast or uterine cancer, blood clots, stroke, heart attack, liver disease or unexplained vaginal bleeding, or who may be pregnant. Report any unexpected bleeding.
